  • A foreign passport in the original
    passport must be valid for at least three months after the end of the Schengen visa for which documents are being submitted;
    the passport must have two blank pages (turn) for gluing the visa;
    the passport must have been issued within the last ten years;
  • Copies of the foreign passport
    , copies of the identification page and all pages with previous visas and stamps (if two valid foreign passports, provide copies of all pages of two passports).
  • Copies of the internal passport and identification code of
    all sixteen pages of the passport;

  • A certificate from the place of work or educational institution for submission to the visa center of the Embassy of Sweden in Ukraine the
    certificate must have the following information: it must be issued for presentation to the visa department of the Embassy of Sweden in Ukraine, indicating the position held, since when the applicant has held this position, monthly salary for the last six months, the duration of the permitted leave and whether the job will be retained during the stay abroad. The certificate must indicate the name and position of the employee who signed the certificate, as well as the contact and registration details (or registration certificate) of the company;
    certificate from the educational institution for students and a copy of the student card.
  • Documents for private entrepreneurs
    copy of the certificate of registration of a private entrepreneur;
    tax declaration (report);
    Bank statement.
  • Documents required for issuing a visa for children under 18 years of age,
    the original and a copy of the birth certificate;
    for a child to travel unaccompanied by parents, guardians or accompanied by one parent or guardian, it is necessary to provide a written consent for a visa and a trip to Sweden, certified by a notary;
    court decision on sole custody, death certificate or other document that confirms this;
    copies of national or foreign passports of guardians and parents;
    for children of school age, a certificate from the school.
  • Insurance policy (health insurance)
    minimum coverage under the policy should be 30,000 EUR.

For applicants who are not citizens of Ukraine,
a copy of a valid residence or employment permit in Ukraine. The original permit must be presented when submitting documents.
